Below you will be asked to identify your instructor and provide your student identification number (T Number). Be assured that your student id is separated from your evaluation the moment you click the submit button below. This additional step is simply to ensure that each instructor receives only one evaluation from each student actually enrolled in the class. Thank you for participating in the evaluation process. Each evaluation is summarized electronically and ONLY the computed results are shared with the instructor AFTER all grades have been recorded.
